Possible Causes

The number of available VTY channels is lower than the threshold.

Procedure

Run the user-interface maximum-vty number command to configure a larger number of VTY channels and run the user-interface vty first-ui-number [ last-ui-number ] command with a larger difference between first-ui-number and last-ui-number, or log out logged-in users.
